Emerging value-based payment (VBP) models are critically important in meeting future needs for improved quality and cost of healthcare. This webinar will feature Dr. Laura Sessums from the CMS Centers for Medicare and Medicaid Innovation’s (CMMI) Seamless Care Models Group. Dr. Sessums will discuss the Comprehensive Primary Care Plus (CPC+) national advanced primary care medical home model, both what it is and how practices are doing the work in the model.
